Please note that the student's Special Education Case Manager is the primary point of contact for the development, implementation, and monitoring of the student's Transition Plan.
The Case Manager works directly with the student, family, school staff, and community partners to ensure that transition goals and services align with the student's postsecondary plans, including education, employment, training, and independent living goals.
If you have questions regarding your student's Transition Plan, transition assessments, goals, activities, or required documentation, please contact your student's Special Education Case Manager directly.
